We collect personal information whenever you provide it to us.
Where you are a client or a prospective client we collect may include the following:

Name, address, date of birth and contact details
Your medical history including current and historic illness, treatment and medication
Any dietary needs you may have
Any allergies we need to be aware of
Name and contact details of authorised persons which you consent for us to contact regarding your care
Financial information

We may also collect anonymised information about your use of our website.

 

3. How we use your data  

How we use your data will depend on the purpose for which you have provided your data:

If you make an enquiry we will use your data for the purposes of responding to your enquiry.
If you register with us to receive news and information about Next Level we will use your information to add to our mailing lists and keep in touch with you.
If you become a client of Next Level we may also use your data to provide our care services to you and also for the purpose of audits to monitor and ensure compliance with our strict information handling policies, for training purposes and to develop and improve our business.

The General Data Protection Regulation (GDPR) 2018 requires the Organisation to
under 6(1)(f) “Processing is necessary for the purposes of the process:
Personal data  legitimate interests pursued by the controller or by a third party, except where such interests are overridden by the interests or fundamental rights and freedoms of the data subject which require protection of personal data, in particular where the data subject is a child.”
Article 6(1)(a)” The data subject has given consent to the processing of his or her personal data for one or more specific purposes.
Sensitive personal data
(Health Records) under 9(2)(h) – “Necessary for the reasons of preventative or occupational medicine, medical diagnosis, the provision of health or social care or treatment or the management of health or social care systems and services.”

 

7. Reasons we collect and use your personal information 

We have a legitimate interest in collecting and using your personal data.  Where you are not a client of Next Level Homecare our legitimate interests are that we use your personal information in order to respond to any enquiries made by you and to provide you with the information that you have requested.
In some circumstances we will seek your specific consent for certain identified uses of your personal information, such as where we add your name and contact details to our email marketing databases. We will make it clear when we are seeking your consent for any particular use of your information. Where you do provide any such consent you can withdraw this at any time by letting us know using the contact details below or simply using the unsubscribe options in our emails.

Data Protection Legislation (GDPR 2018, DPA 2018) gives you a right to access the information we hold about you on our records. Requests must be made in writing to The Data Protection Officer, Next level Homecare, Suite F 2 Bakers Yard, 163 High Street, Uxbridge, Middlesex, UB8 1GG.  We will provide your information to you within one month (this can be extended dependent on the complexity of the request) from receipt of your application:

A completed application form, containing adequate supporting information (such as your full name, address, date of birth) to enable us to verify your identity and locate your records.

Information will be provided free of charge except where requests are unfounded or excessive, repeat requests then we may either charge a reasonable fee or refuse to act on the request
You have rights including to:

Require us to correct any inaccuracies in your information free of charge

To require us to erase personal data that we may process about you where this is no longer required to be processed by us

To object to or restrict our processing of some of your personal information in certain circumstances

We will retain your information for no longer than is necessary for the purposes for which we collected it, or for as long as we have your consent to do so where your consent is our legal basis on which we process such data.

As a result, our retention policy differs significantly based on the purposes for which the data was collected and the nature of the data but by way of general guidance we will usually retain your data:

For a period of 3 months following your last interaction with us where you have made an enquiry

Until such time as you unsubscribe from any mailing lists that you may subscribe to

For a period of up to 6 years from the date you cease to be a client of Next Level Homecare

Our website contains links to other websites of interest. However, once you have used these links to leave our site, you should note that we do not have any control over that other website. Therefore, we cannot be responsible for the protection and privacy of any information which you provide whilst visiting such sites and such sites are not governed by this privacy statement. You should exercise caution and look at the privacy statement applicable to the website in question.

We will not sell, distribute or lease your personal information to third parties unless we have your permission or are required by law to do so.

Raising a concern

If you have any concerns about how we handle your information you have a right to complain to the Information Commissioners Office about it.
